Why I Created Elevate
I believe exceptional healthcare begins with something simple: taking the time to know the person behind the patient.
I am an Autonomous Nurse Practitioner with more than six years of advanced practice experience and over twelve years as a Registered Nurse. I hold a Master of Science in Nursing as a Family Nurse Practitioner and a Bachelor of Science in Nursing.
Throughout my career I have cared for patients across a wide range of clinical settings, including primary care, oncology, hematology, emergency medicine, critical care, trauma, and emergency medical services. My experience has continued to expand into IV therapy, hormone optimization, peptide therapies, medical weight management, performance and recovery, and personalized wellness care.
Over the years I began to recognize something that ultimately inspired the creation of Elevate Performance and Innovative Care. People want more from their healthcare experience. They want time to ask questions, understand their options, and feel heard. Most importantly, they want care that considers who they are, how they live, and what they hope to achieve.
That is why I created Elevate.
My approach is intentionally personal. I want to understand your health history, your concerns, your lifestyle, and your goals before determining the next step. Rather than beginning with a predetermined treatment or a fixed menu, I believe care should begin with a conversation.
Whether you are exploring hormone optimization, medical weight management, peptide therapies, longevity and wellness services, or personalized IV therapy through revIVe, my goal is to help you understand your options and develop an individualized plan when clinically appropriate.

MEDICAL DIRECTOR
Bradley Homan, DO, FAOAO
Orthopaedic Surgeon · Sports and Regenerative Medicine
An orthopaedic surgeon with extensive experience in sports medicine, regenerative medicine, joint replacement, and reconstructive surgery. A Florida native, born in Tampa, he lives in Orlando with his wife.
Founder and president of Celebration Orthopaedic and Sports Medicine Institute, Dr. Homan has built a career centered on helping patients restore function, improve mobility, recover from injury, and return to the activities that matter most to them. He developed the Sports Medicine Department at AdventHealth Celebration and previously served as Chief of Surgery. He is a team physician for nine area high schools and has worked with athletes competing in the NFL, NBA, MLS, NCAA, and Disney endurance events.
A significant focus of his work is regenerative medicine, an evolving area focused on supporting the repair and restoration of damaged tissue and function. He earned his undergraduate degree with honors from the University of Florida and his medical degree with honors from Nova Southeastern University, completed his orthopaedic surgery residency through Ohio University, and pursued fellowship training in sports medicine and arthroscopy.
